Full Job Description

This role will require rotations throughout the services whilst carrying out accurate occupational therapy assessment and treatment of patients with diverse clinical presentations within an acute and rehab setting, and complex physical and psychological needs. You will need to implement clear, clinically reasoned and appropriate individualised treatment plans which are regularly reviewed and updatedensuring effective and consistent communication with all parties involved. The successful candidate will work collaboratively as part of the multidisciplinary team with patients, their families, consultants and agencies to proactively plan and manage a patientdischargeneeds as early as possible. This includes early identification of complex care needs and liaising with relevant teams.
