Full Job Description

An exciting opportunity has arisen for a Band 6 Occupational Therapist to work within the our friendly and supportive Calderdale Community Rehabilitation Teams on a permanent basis, up to 37.5 hours/week available.

The Community Rehabilitation Team is a multi-disciplinary team, and consists of Physiotherapy, Occupational Therapy, Speech and Language Therapy, Dietetics, Falls Prevention Workers and others support staff. The different disciplines work together in an integrated manner to provide the best treatment for patients with a range of acute and long-term conditions.

The majority of clinical work takes place within patient's homes (including care homes). However, we also see patients in clinics or groups when it is more appropriate to do so. At times, the team visits patients in hospital to participate in meetings, or to assist complex discharge planning. Therefore you will require access to transport with business insurance., To have specialist assessment and treatment skills in the areas listed but not limited to:

  • Cognitive and perceptual deficits

  • Falls management

  • Neurological conditions

  • Early dementia

  • Anxiety management

  • Complex psychosocial assessment of patients and carers

  • Assessment for assistive equipment and adaptations

  • Assessment of the home environment

  • Fatigue management

  • Self-management


  • Determine a clinical diagnosis, develop, deliver, implement, evaluate and modify individualised treatment programmes and/or provide specialist advice, ensuring appropriate pathways of care and communication via liaison and referral to other agencies as required
