Our Acute Assessment & Frailty Team are now recruiting and we want you to be part of our next exciting development! Due to continued investment and service development within the team, we're recruiting Band 6 Occupational Therapists or Physiotherapists. We believe in patients receiving the right care, in the right place, at the right time and will support our patients to safely avoid an admission to hospital wherever possible. Our patients are some of the most vulnerable in our community and we're here to support them when they need us most. Sometimes a friendly face and listening ear is all it takes to make a difference! Our team is integrated, consisting of occupational therapists, physiotherapists, therapy assistants, therapy apprentices , nursing colleagues and doctors, meaning you will always be supported to provide holistic therapeutic interventions, and someone will be on hand to support with decision making. You will have regular training and CPD opportunities to ensure you continue to develop your clinical skills and will have the opportunity to develop your leadership skills through the supervision of clinical staff members such as apprentice's, therapy assistants and students. The service provides 7 day working and extended hours across both the Preston and Chorley sites, therefore the successful candidate will be required to support these working hours. No day, in fact no hour is the same within this team. Covering multiple areas, from the Emergency Department's to the Acute Frailty Unit you will certainly be kept busy, relying on your excellent organisational and prioritisation skills. You will often be faced with challenges, however as a keen and proactive problem solver you will quickly see how you're making a difference to patient's lives. As a specialist occupational therapist or physiotherapist your excellent assessment skills and timely decision making will support patient flow and improve patient experience by ensuring patients are not admitted to hospital unnecessarily. You will undertake robust, holistic, expert assessments and interventions as well as identifying the safest pathway for patients to be discharged from the emergency department or other assessment units. Your caseload will vary on a daily basis and you will draw upon your previous clinical experiences to support you in delivering efficient and effective care. Active involvement in continuing professional development is a key requirement, alongside a commitment to promote clinical governance and evidence best practice for this speciality. To be great at this job you need to be passionate about delivering excellent patient care whilst also having an awareness of trust and community systems and pathways to guide your decision making. Your individualised assessments will keep the patient at their core, identifying what matters most to them in order to support them to safely leave the acute setting. Excellent written and verbal communication skills are vital in such a fast paced, busy environment. It's important to ensure that all channels of communication are clear and effective to both wider members of the MDT and the patient. Excellent interpersonal skills, an aptitude for team working and being able to support the Trusts values and behaviours are also important in your success. There will be training opportunities for you to develop your knowledge both formally and informally. We also have a profession specific band 6 forums throughout the year, where you will receive peer support from your colleagues as well as in-service training within the immediate and wider teams. Duties will include but not limited to:
- To communicate and collaborate with all Integrated Frailty Team / Admission Avoidance stakeholders
- To support personal and people development
- To help ensure the health, safety and security of users of the Acute Assessment and Frailty Team service, self and colleagues
- To support with service development and planning
- To support the Clinical Governance and Quality agenda within the Acute Assessment and Frailty Team
- To support equality of opportunity and access within the Acute Assessment and Frailty Team for both staff and patients
- To support financial and other resource management within the service
- To directly support the delivery of therapy to patients
