Full Job Description

We are looking for a Band 7 Specialist Older Adults Psychiatry Liaison Practitioner to join our Psychiatry Liaison Service at St Peter's Hospital in Chertsey, Surrey.

The successful candidate will be involved inundertake initial assessments and follow up reviews of older people presenting with mental health issues and/or dementia at St Peter's Hospital (currently inpatient wards and A+E).

They must be able to work flexibly within the team and provide supervision to a team of nurses. The posts require shift working.

Informal visits are welcomed and encouraged., I will provide expert interventions and oversight to the care of older people within the acute general hospital pathway. I will ensure that the care delivered is excellent and in keeping with the most up to date guidelines. I will have a good knowledge of mental health within working age adults.

I will support the growth and development of other members of the liaison psychiatry team whilst supporting the service manager and wider Trust to achieve an exemplary service underpinning our visions and values. My role will include providing education, challenging stigma and actively supporting service improvement and development.

I will put the experience of the person using the service at the centre of my decision making whilst building and maintaining supportive and responsive relationships with colleagues in the acute trust.

I will build outstanding relationships with all relevant services and will lead on clinically challenging and complex cases
