Full Job Description
The post holder will be an autonomous practitioner whilst working as part of the Tees Specialist Palliative Care Multidisciplinary Team (MDT) in partnership with other specialist palliative care providers within the South Tees Locality. The post holder will work closely with and within the multi-organisational, multi-professional arena with statutory and non-statutory organisations including some cross boundary work within the North of England Strategic Clinical Network (NESCN), the Acute Sector, Social Care, Hospices, Care Homes and Teesside University. The Specialist Nurse will provide high quality specialist palliative care (direct and indirect) and advice to patients, carers and professionals in line with Macmillan's four Levels of Intervention for the surrounding population. Clinical leadership and support will be provided by the Clinical Matron, Nurse Consultant and Cancer Lead.

You will be expected to attend the leadership development programme, and the Quality Improvement Programme that the Trust delivers. This programme aims to

- Practitioner level in quality improvement, equipping you with the skills to champion, lead and complete quality improvement within your role After you complete this four and a half days of training you will be able to explore further leadership and improvement training opportunities, we offer further in-house courses dependent upon your role and bitesize programmes and leadership apprenticeships at level 3, 5 and 7.
