Are you passionate about making a difference to those needing access to Community Specialist Palliative and End of Life Care? We are delighted to be able to offer the opportunity to join our Community Specialist Palliative Care Team working across the Sutton and Merton boroughs. We work very closely with patients and those important to them, offering complex pain and symptom control, psychological and emotional support . We offer advice to GPs, District Nurses and other healthcare professionals as well as sign posting to other services where appropriate., We are currently seeking to recruit a Band 7 Community Clinical Palliative Care Specialist and would also consider a Band 6 Development Post for the ideal candidate who demonstrates passion and commitment. We welcome and encourage informal visits, please contact the HR department or Tracy Christmas Community Services Manager on , who will be happy to assist Work as a specialist palliative care resource in relation to the control of symptoms and the provision of social, economic and spiritual care for the patient and family in the community setting. To provide advice, support and education to the primary health care team where appropriate, as part of the multi-disciplinary team.

  • Are you a self-motivated and experienced nurse or qualified healthcare professional who is able to join our well established multi-disciplinary Community Specialist Palliative Care Team?
  • Can you demonstrate skills, knowledge and experience of working with patients with specialist palliative care needs?

    We are a 13 bedded hospice, serving the communities of Merton and Sutton by supporting those with life limiting conditions to live their lives as fully as possible. Caring for people at the end of their lives can be challenging, but it can also be immensely rewarding.
