Full Job Description

+ Working in the anticoagulation clinic dosing outpatients and inpatients as part of our anticoagulation team
+ Supporting rotational pharmacists and the anticoagulation technician in delivering the anticoagulation dosing service
+ Providing clinical pharmacy cover to our critical care units and to cardiac services, including team leadership to the cardiac pharmacy team where needed
+ Participation in Saturday, Sunday, bank holiday and late working pharmacy rotas.
+ To provide an emergency call out service on a rotational basis.
+ Our Saturday, Sunday & Bank Holiday shifts are part of normal contracted hours with compensatory 'off-duty days', on a rota basis of roughly one weekend day every 6 weeks, the welfare of children, young people and vulnerable adults. This is an exciting split post that encompasses anticoagulation, working on the cardiology and cardiac surgery wards, as well as critical care. It is a flexible position that can be adapted to suit you and your experience so far, and there will be plenty of learning opportunities from each team.
You will be working in a friendly team within the pharmacy department to provide anticoagulation dosing advice to patients being managed within both primary and secondary care. You will also provide specialist input to adult cardiac areas and critical care as part of the clinical pharmacy service, and there will be an opportunity to deputise for lead anticoagulation and cardiac pharmacists as required.
Please note, this is a Fixed Term position.
