Full Job Description

Provides specialist pharmacy role within the multi-disciplinary team at St Mark's Hospitals.

Participating and contributing to IBD ward rounds.

Ensuring outpatient and inpatient pharmaceutical care for inflammatory bowel patients are in line with national and international standards.

Working with homecare companies in line with the professional standards of homecare service to ensure patients are discharged and managed at home appropriately and to a high standard.

Helping to deliver and develop pharmacy services to St Mark's Hospital in accordance with appropriate Trust objectives in conjunction with the St Mark's Pharmacy Manager.

Takes part in education and training of healthcare professionals within St Mark's and nationally.

Undertake audit and service development to further improve the pharmacy service to St Mark's patients in conjunction with the multi-disciplinary team.

Provide a clinical pharmaceutical service to an allocated ward(s) as part of a team.

Support with IBD clinics, including the immunomodulatory therapy blood monitoring clinic
