Detailed job description and main responsibilities To deliver a clinical service to patients, including taking patient drug histories, rationalising prescribing, advising on and compiling treatment protocols, therapeutic drug monitoring and monitoring compliance.To support the CMG Lead Pharmacist in maintaining and improving the pharmaceutical service to the clinical management group (CMG). To be responsible for the delivery of an agreed standard of pharmaceutical care to identified patients in order to achieve the safe and cost effective use of medicine Please see the attached Job
