Full Job Description

Community Stroke Rehabilitation in County Durham are looking for a qualified Physioherapist with experience in stroke rehabilitation to join our successful team. The available post will be based at Chester le Street Hospital, covering the surrounding areas. The successful candidate will work alongside Occupational Therapsits, Speech and Language Therapists, and skilled associates to deliver innovative, best practice, evidence-based stroke rehabilitation in patients own homes and out-patient settings to optimise recovery., Carry out intricate neuro assessments with patients within their own environments. Identify goals with patients, conduct reviews, create exercise programmes and monitor ongoing rehabilitation.

Conduct supervisory duties with other team members and contribute to CPD

Feed into weekly team MDT

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
