Full Job Description

We are seeking a motivated, compassionate, and dynamic Physiotherapist with experience in elderly care and comprehensive health needs assessment to apply for this exciting opportunity to work as a Specialist Physiotherapist in the Community Ageing Well Service in Barnet.
The service is funded through Ageing-well and is focused on providing proactive holistic / comprehensive geriatric assessments. The aim is to facilitate coordinated care for those with complex health needs and ultimately reduce unplanned hospital admissions.
The team coordinates and chairs a virtual MDT twice a week with the core team and a geriatrician from BGH/ RFH and monthly with an old age psychiatrist from BEH., Be an active member of the Community Ageing Well service, providing a clinically comprehensive Physiotherapy service and anticipatory care for adults 65+ living in the Borough of Barnet. As part of this Ageing well-funded service you will provide proactive holistic patient centred assessment, treatment, and care to minimise unplanned care hospital admissions, optimise patient's health conditions, facilitate care closer to home, improve patient and carer experience, reduce avoidable harm (PU/ Falls) and to increase the number of patients dying in their preferred place care., The post holder will be responsible for:

  • Undertaking specialised uni-professional and multi-professional assessments a part of necessary to the comprehensive geriatric assessment, and re-assessments of treatment programmes using evidence-based outcome measures and in accordance with CSP Guidelines.

  • Working within professional standards and clinical guidelines, promote best practice and to undertake all aspects of clinical duties as an autonomous practitioner including assessing and treating own caseload of patients/clients and maintaining professional documentation.

  • Being responsible for the supervision of junior staff, rehabilitation assistants, physiotherapy students, Frailty MDT Administrators, and Frailty Support Workers.

  • Frailty Clinicians working within the organisation may at times be required to work with other teams, from other bases or localities.

  • Supporting in the operations of the team.
