Full Job Description

This is a full-time role, working 37 hours per week.

Day to day duties of the role

-Proactively delivery the Specialist Support Model of inclusive practice and implement the SEND Code of Practice

-Planning and reviewing individual programmes of support ensuring all aspects of provision and support specified within a learner's ILP and/or Education Health and Care Plan is in place and the progress is effectively tracked and monitored

-Carrying out high quality Initial Needs Assessments and diagnostic assessments along with other appropriate assessments to establish support needs and effectively track progress leading to successful outcomes

-Arranging and deliver transition programmes to ensure seamless transitions between FE and HE

LDD Specialism e.g. foundation degree in LDD or other specialist qualification.

-Working Knowledge of Additional Support Funding

-Relevant and Successful experience in learning support

-Skills in the development and implementation of Individual learning programmes, Disability Confident
