Full Job Description
We are seeking a Mental Health Nurse, Social Worker or Occupational Therapy team leader to join our developing primary care Mental Health Team in the East of Southampton. You will be based within GP surgeries and working with service users of adult working age including older persons with mild to moderate mental health difficulties. You will work within scope of practice so if have worked within specialities these skills will be transferable., The team leader role is largely clinical; you will contribute to the triage service; offering early assessment, signposting, brief interventions, group deliver for people with mental health needs in their local communities. You will play an active part in the development of systems and processes which support care delivery within these teams. You will require good communication skills and be able to build positive partnerships with key stakeholders in primary and secondary care. You will work collaboratively with Solent Mind and have peer recovery workers work alongside practitioners in the surgery.
The successful applicant will become part of a growing primary care team who deliver care to some of the most diverse populations in Southampton. Our primary care setting is a rewarding place to work. We are looking for an enthusiastic and creative person who is committed to maximising life opportunities for our patients and their families. The PCN is a large, inclusive team with a focus on multi disciplinary working. You will need to have a flexible, multi-disciplinary approach, have a good understanding of risk assessment, crisis planning and the recovery model., The role requires excellent communication skills, high standard of risk assessment skills, partnership working and being able to work in a clinic environment within the GP surgeries.
The new workforce with this orientation can create a psychologically informed environment that supports and encourages, while acknowledging distress and motivating work on change and addressing challenge.

£46,148 to £52,809 a year already an existing post 30 hours - someone leaving
