Full Job Description

We are looking for an enthusiastic SLT to come and work with us in West Hertfordshire, in our Integrated Community Stroke Team (ICSS) and Neuro Enhanced Transition Team (NETT), for a year to cover maternity leave The ICSS provides assessment and rehabilitation for adults, from 18 years upwards, who have had a Stroke in both home and out-patient clinic settings. We work closely with other healthcare professionals, services and agencies from across the whole Stroke pathway and wider CLCH adult community services portfolio. Working within a cohort of SLTs, this post creates a great opportunity for someone who is conscientious and adaptable to deliver forward thinking, progressive stroke rehabilitation to patients primarily in their own home, but also in a clinic environment. The successful candidate will have excellent communication skills and be able to demonstrate an adaptable approach whilst meeting guidelines and stroke standards. You will form an essential part of a dedicated, knowledgeable multi-disciplinary team that regularly engages with team and individual clinical supervision. Development and training opportunities are available through the CLCH Academy. You will maintain responsibility for an individual clinical caseload within the wider ICSS team supporting people with communication and swallowing difficulties. There are opportunities to be involved in developing the newly enhanced service, including delivering group interventions. You will form an essential part of a dedicated, knowledgeable multi-disciplinary team that regularly engages with team and individual clinical supervision. Development and training opportunities are available through the CLCH Academy. You will maintain responsibility for an individual clinical caseload within the wider ICSS team supporting people with communication and swallowing difficulties. There are opportunities to be involved in developing the newly enhanced service, including delivering group interventions. Patients are seen in their own homes or community settings, with a recently renovated office and outpatient base at Harpenden. All team members are fully agile working, enabling blended in-person and virtual sessions to be delivered as best meets client needs. As such, the post holder will be able to work flexibly from home or the office in a pattern negotiated according to their circumstances and service needs. These clients will present with a variety of complex neurological conditions, but with an emphasis on those recovering from stroke. To analyse auditory, visual and kinaesthetic aspects of client's communication and to identify appropriate strategies to facilitate and enhance communicative effectiveness in the home, at work and other relevant settings. To carry out specialist neuro-physical examination of the orofacial process including the mechanism of eating and swallowing. To monitor all aspects of the interaction of significant others with clients. In particular to simultaneously monitor auditory, visual and kinaesthetic aspects of dyadic interactions, encouraging adaptive strategies to maximise effective communication
