Full Job Description

The post holder will be expected to carry out general duties alongside their clinical duties which will include the administrative duties associated with the care of patients. They will record clinical activity accurately and comprehensively and submit this promptly to the Information Department. They will also be expected toparticipate in service and business planning activity for the locality and, as appropriate, for the whole mental health service, includingparticipating in annual appraisals.

Clinical Duties of the post holder

The post holder will be expected to give support to the consultant and the ADAPT team in developing and maintaining the caseloads of all patients ensuring that Core Assessments, Risk Assessments and Care Plans are available in RiO case records. To provide reviews including Mental Health Act where necessary to the ADAPT team and to develop comprehensive case-note summaries, comprehensive multi-axial assessment processes and care planning in line with NICE guidelines.

The post holder will also be expected to attend ADAPT team meetings and to ensure the involvement of families and carers, wherever possible, with the patient's consent in treatment planning. To always support the consultant in preparing any necessary reports, and attending Mental Health Review Tribunals if applicable.

They will also provide clinical cover for the consultant, when she/he is away on leave, with supervision by another consultant.

The ADAPT pathway will offer a flexible service to service users presenting with complex presentations (excluding Psychosis or Bi-polar disorder) who currently require secondary mental health care. Presenting problems will include Anxiety, Depression, Personality Disorder and Trauma. Referrals to ADAPT may come from Primary Care Plus via triage, from the Wards, Crisis or Day Treatment Teams, EIP teams (following clarification of diagnosis), Forensic services or because of Out of Area transfers.

The caseload of the ADAPT team fluctuates but on average is about 400 to 450 patients. It is expected that the post holder will have regular meetings with the team manager to understand referral patterns, trends in the case load size, delays in transfer of care or discharge to primary care and waiting times for appointments for crisis clinics or initial assessments.
