Full Job Description

The advertised post is for a Speciality Doctor post in Geriatric Medicine, based in St David's Hospital and Barry Hospital. Sam Davies ward in Barry Hospital, together with Lansdowne, Elizabeth and Rhydlafer wards based at St David's Hospital provides ongoing community hospital based in-patient care for frail older patients
The patient case mix largely consists of frail older people who have ongoing chronic disease management needs as well as recovery, rehabilitation, reconditioning and complex discharge planning needs. Working in partnership with the acute hospital sites and Rehabilitation liaison nurses and Consultant Geriatricians at both sites, patients are identified at the earliest point of their acute journey who no longer need acute hospital care. The aim of the wards is to provide a holistic, multidisciplinary, rehabilitation driven and ongoing care service to frail older patients, in order to maximise their functional independence, psychological wellbeing and discharge from hospital in the overall context of 'Chronic Disease Management'. In addition, the wards care for a cohort of severely frail patients whose needs often encompass an End of Life Care approach but who may have needs that are too complex, intense, unstable or unpredictable to be met at home or in a care home setting.
You will join a team of Consultant Geriatrician, Specialty Doctor and wider Nursing and MDT Colleagues in delivering this model of care

Main duties of the job
The post holder will have a commitment to be available for 8 sessions p/w. They will need to demonstrate professional excellence and the ability to deliver effective care. He/She will need to agree levels of activity and throughput with the Clinical Lead and undertake annual appraisal and job planning as per UHB Policy. The post holder will be required to adhere to UHB Policy on maintaining Medical excellence and to be committed to maintaining their standard of performance by keeping knowledge and skills up to date. Inpatient ward duties and ward rounds, to include review of and acting upon investigation results. Comply with GMC guidance on Good Medical Practice. Helping ensure patient admissions run smoothly, efficiently and effectively by encouraging multi-disciplinary team working. Ongoing training of junior staff by giving feedback on case presentation, examination techniques and diagnosis.Carry out teaching duties as required and take an active part in the
undergraduate and postgraduate medical education programme and in the teaching of junior staff. Active participation in departmental meetings. Participate in service development and improvement through commitment to clinical governance. Take part in and lead as appropriate team, departmental and regional audits. The post-holder will be expected to organise and participate in audit both related to both clinical and organisational practice.
