Full Job Description

  • Conduct Care Act assessments for individuals with frailty and mental health issues.

  • Develop and implement care plans, and regularly review services provided.

  • Manage a busy caseload with a high volume of referrals.

  • Assist with hospital discharges, including those from hospitals outside the St. Helens area.

  • Undertake safeguarding investigations in line with Local Authority procedures.

  • Ensure compliance with current legislation and government policy, particularly the Mental Capacity Act and DoLS.

    St. Helens Metropolitan Borough Council is seeking a skilled and experienced Adults Social Worker to join our Frailty & Mental Health Team. This crucial role involves working with individuals aged 67+ who are dealing with age-related frailty and mental health challenges, including dementia., Qualified Social Worker with experience in adult social care, particularly in working with the elderly.

  • Strong understanding of relevant legislation, including the Care Act, Mental Capacity Act, and DoLS.

  • Ability to manage a high caseload and work effectively in a busy team.

  • Experience in conducting Care Act assessments and developing care plans.


  • Team Environment: You will be part of a large, supportive team of 15 Social Workers and 15 Assessment and Review Workers. The team benefits from strong support structures, with regular supervision provided by one of three team managers or assistant managers.
