An exciting opportunity has arisen to join our Patient & Carer Wellbeing Service (PCWBS). The PCWBS provides a blended model of telephone support, outpatient and face-to-face services.

This is a part time role working 4 days per week. You will work at our Coventry Hospice and travel to our Warwick and Rugby sites when required.

If you would like to be part of a motivated, forward-thinking team, who are passionate about providing high-quality care and support to people living with or caring for someone with a life-limiting illness - apply now., Working closely with a multi-disciplinary team, you will coordinate the assessment, planning and evaluation of individualised, patient-centred care, with a holistic approach that is sensitive and responsive to the ever-changing needs of patients, families and carers.

• ENB 998 or equivalent teaching and assessing qualification


• Evidence of continual professional development


• Significant post-registration experience in:


1. Needs assessment / care planning
2. Palliative and end of life care
3. Assessment and symptom management


• Skills in communication and organisation


• The right to work in the UK (we cannot help with visa sponsorship)


Because you will be working with patients and their families, we will carry out an enhanced DBS check including the adults' and children's barred lists.
