Full Job Description
Acting as a named nurse within a multidisciplinary team, developing collaborative and needs based care plans, through assessment, planning, implementation and evaluation.
Providing excellent patient centred, evidence based nursing care.
Actively be involved in maintaining the wellbeing of patients and colleagues and working in a collaborative manner with all members of the Multi disciplinary team.
Being visible and accessible to carers, patients and families ensuring safe and efficient caring services adhering to the Trusts Triangle of Care initiative.
Contribute to achieving all performance indicators, essential standards and quality outcomes., The role of the Band 5 Staff nurse is to carry a caseload of patients whom they will support through the patient pathway at the Day Hospital. This will entail the completion of all appropriate documentation, liaison with family and care agencies, liaison with the multi disciplinary team and referrals on to supporting disciplines.
The nurse will also be expected to spend time with their patient building up a therapeutic alliance and tailoring care plans to each individuals needs and a bespoke treatment plan alongside senior nurses/medics.
The Castleside Day Hospital is a mental health unit providing assessment, treatment, therapeutic intervention and rehabilitation for the older adult experiencing mental health difficulties such as psychosis, severe depression and dementia.
The Day Hospital is classed as planned care and sits between the community treatment team and the inpatient units. We provide increased support for patients whose needs may not be able to be managed alone in the community or to prevent admission and we also take patients who have been an in patient but wish to return home and still require substantial risk management., We aim to attract and retain a diverse, talented and committed workforce, who are caring and compassionate, and therefore able to meet the demands of the modern NHS now and in the future.
