Full Job Description
Brook House is a small care home located in the village of Ottershaw, near Woking. Conveniently situated close to Junction 11 of the M25, it is approximately a 20-minute drive from Woking town. Brook House operates as a care home with nursing, supported by a dedicated team including a Home Manager, Nurses, and Band 2 and 3 Healthcare Support Workers. It is registered with the Care Quality Commission (CQC).
Brook House provides care for four residents with a range of needs, including learning disabilities and neurodiversity. Each resident has their own flat, making Brook House a true home tailored to their individual needs.
This developmental role offers a variety of clinical duties, including medication administration, monitoring both mental and physical health, and supervising junior staff.
You will support four adults with learning disabilities in a care home located in Ottershaw, near Woking in Surrey. Responsibilities include care planning, physical health monitoring, and ensuring the effective administration of medication., We are seeking a passionate and dedicated Registered Learning Disability Nurse to join our team. In this role, you will help support our residents in living fulfilling lives and enjoying the opportunities available to everyone in society. Our residents love going on holidays, dining out at restaurants, and visiting the pub. With their diverse interests, you will play a key role in supporting them to pursue and enjoy these activities.
As a Registered Nurse, you will lead shifts, ensuring the team provides exceptional support to residents in living their fullest lives. You will report directly to the Clinical Lead and Home Manager while also collaborating with the Service Manager.
We are committed to your professional development within the Learning Disabilities Division. You will be supported by a Clinical Lead and Manager, both of whom are also registered nurses, as well as by ongoing training and development opportunities provided by Surrey and Borders.

To positively contribute to divisional developments in relation to nursing practice.
+ To advocate for the residents and represent the service.
+ To contribute to the supervision and Individual Performance Review/Appraisal of support workers.
+ To contribute to the development and implementation of appropriate policies, protocols and care pathways.
+ To contribute to initiatives to ensure that the needs of people with protected characteristics are addressed within local services and future developments.
Clinical
+ To demonstrate specialist nursing skills in areas associated with learning disability such as crisis intervention, alternative communication, the assessment and management of people with additional mental health needs, epilepsy, medication management, challenging behaviour and other conditions.
+ To actively involve people in developing and agreeing their own care plan, to act as an advocate and ensure that there is proper regard for their dignity, choice, self-esteem, racial, cultural, sexual and religious needs and preferences.
+ To carry out comprehensive assessments, plan implement and evaluate delivery of nursing interventions as part of a holistic approach working in a variety of settings.
+ To practice within the Positive Behaviour Support framework.
+ To undertake comprehensive risk assessments and develop effective risk management plans.
+ To work in partnership with acute and primary care services and other healthcare providers.
+ To establish and maintain effective relationships with colleagues, other agencies, people who use services and their families.
Professional
+ To be professionally and legally responsible and accountable for all aspects of clinical work and care. To maintain high standards of care in all areas of practice in accordance with the NMC Code of Conduct, professional guidelines and Trust Policies and Procedures. To act within the Mental Health Act and Mental Capacity Act (2005).
+ To be aware of and implement Safeguarding Adult and Child Protection Policies, Procedures and legislation. To carry out professional responsibilities under the Mental Health Act 1983 and other relevant legislation.
+ To contribute to conducting clinical audit and research, maintaining current knowledge of research findings and development within the learning disability field.
+ To participate in quality assurance activities as required.
+ To contribute to the development, implementation and monitoring of interventions driven by government initiatives e.g. Transforming Care, LD Mortality review to promote positive health gains for clients.
+ To contribute to developing and providing a range of specialist teaching/training packages which can be delivered to a range of multi-disciplinary audiences.
