Full Job Description

Fixed term Maternity leave cover 12 month contract 22.5 hours a week, The main duties of this post are:
+ Delivering complex wound care over several locations to ambulatory patients
+ Managing complex Leg ulcer patient in line with Trust policy
+ leading on MDT clinics, where we proactively manage patients wound care
+ Liaising with the wider MDT including DN's, Podiatrists, TVN, Heart Failure and Vascular Nurses, Because we are an innovative, integrated Community service covering the East Staffordshire area including the Burton Town area, Uttoxeter and the surrounding Villages. We work closely across the Nursing teams, AHP's including Podiatry, Physio, Falls Team, Stroke and Rehab team including our out of hours service, to ensure that our patients are at the centre of the care provided. We collaboratively deliver our care across all the professions, recognising the importance for patient centred approach, working together across all the nursing and therapy teams

If the answer is yes, then joining this exciting opportunity to join our Ambulatory Wound Clinic at MPFT in East Staffordshire could be the perfect career move for you. This service runs over 7 days in the Uttoxeter and Burton area providing complex wound care for ambulatory patients
We are looking a qualified nurse with Passion and experience in wound care, with enthusiasm for delivering high quality person centred care in line with the Trust's organisational vision and values.
With strong communication and organisational skills and a flexible approach, you will be expected to work collaboratively with patients and key partners to facilitate the best possible patient experience and outcomes. We are looking for a nurse who demonstrates passion and ability to assess, plan, provide and evaluate evidence based nursing care for patients.
Strong clinical reasoning and prioritisation skills will be required and also the ability to work effectively, both independently and as a member of a team. We are looking for a Nurse who will organise and prioritise their own day- to-day work tasks and activities and delegate tasks to junior colleagues. Also a commitment to professional development and contribute to a culture of continuous improvement is key, Communication & Interpersonal Skills

Education and Qualifications
Essential
+ Registered Nurse with NMC
+ Evidence of professional development
