Join our Children's Community Nursing Team (CCNT) in Oldham on a 12-month fixed-term contract to cover study leave. This is your chance to be part of a dynamic team responsible for providing essential care closer to home for babies, children, and young people facing various health challenges-from short-term illnesses to chronic conditions and even end-of-life care.
As a valued member of our team, you will collaborate with local partners, including community, acute, and tertiary care providers, as well as local authorities and volunteer organisations. You'll be instrumental in delivering holistic, family-centered care, making a difference in the lives of those we serve.

As part of our dedicated team, you will partner with parents and caregivers to provide tailored support for every child's unique needs.
In this dynamic role, you'll monitor and recognize acute illness exacerbations, initiate critical nursing interventions, and collaborate closely with senior clinicians for optimal care. Your expertise will help shape individualized care programs through ongoing assessment and evaluation, ensuring effective communication with multidisciplinary teams.
You'll empower children on their journey to self-care, guiding them toward healthier lifestyles while educating parents and caregivers about health promotion and specific clinical needs.
Additionally, you'll connect community services with children's inpatient units, facilitating smooth transitions and safe discharges. Your responsibilities will extend to community clinics, outpatient appointments, discharge planning meetings, and safeguarding discussions, ensuring vital information flows seamlessly among all parties.

Please note an NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.
