Full Job Description

Adynamic dedicated nurse is required to compliment our current team of staffwithin our intermediate care bed base. Providing rehabilitation for both stepdown and step up patients you need to have a good all round skill set to supportthe needs of these patients. Workingas part of a dedicated integrated health and social care team you will developa variety of skills. A foundation of basic nursing skills is required. Main duties of the job Ensuring ahigh standard of care with intermediate care bed based facility. Liaises with the IntermediateCare Sister / Charge Nurse to ensure information in relation to patientcare / changes to care plan and the continuity of care. Provides mentoring, teaching andassessing to support staff. Liaises withall health care professionals and other agencies to ensure effective servicedelivery in the intermediate care facility. Perform comprehensive assessmentof patient nursing needs. Assess, plan, implement andevaluate programmes of care according to changing health needs under thedirection of the Intermediate Care Sister / Charge Nurse. Maintain accurate records inaccordance with NMC. Attend a multi-disciplinary caseconference as the named nurse if appropriate. To communicate and collaboratewith other members of the Integrated Intermediate Care Team, PHCT, voluntaryand statutory organisations to support individual patient care. To establish and maintaineffective communication with patients, relatives and carers in complex andpotentially challenging situations. To competently perform clinicalskills related to the post. To be professionally responsiblefor delegated visits under the supervision of the Intermediate Care Sister / Charge Nurse, Principle Responsibilities Ensuring a high standard of carewith intermediate care bed based facility. Liaises with the IntermediateCare Sister / Charge Nurse to ensure information in relation to patientcare / changes to care plan and the continuity of care. Provides mentoring, teaching andassessing to support staff. Liaises withall health care professionals and other agencies to ensure effective servicedelivery in the intermediate care facility. Managerial To beresponsible for work allocations of support staff. To complete monthly supervisionsfor support staff. To be responsible for organisingand prioritising own caseload. Leadership Act as a mentor forpre-registration nursing students. Support the development of thenursing team by teaching and assessing members of the team including nursingassistants. Undertake mandatory in-housetraining and other training that supports the role. Access clinical supervision. Undertake training that isspecific to the post. Teaching of patients / carers and otherprofessionals to support patients to maintain their optimum level ofindependence Clinical Perform comprehensive assessmentof patient nursing needs.
