Staff Nurse - Permanent

+ To be responsible for a safe discharge of our patients from the discharge lounge to their destination.
+ Making sure discharge letters and medications are with the patient before discharge
+ Patient centred care, assessing individual patient care needs and developing, implementing and evaluating plans of care for a group of patients in accordance with agreed policies and professional guidelines.
+ Transferring patients from the wards to the discharge lounge in a timely manner to help with capacity and flow.
+ Making sure all referrals are made for our patients who need them.
+ To take charge of the ward for a span of duty, normally working under the supervision of senior staff.
+ To work flexibly to meet the needs of the service.
