Full Job Description

As a Staff Nurse you will provide clinical and professional leadership, supervision and support to patients, and students within the SMART team.
You will triage, assess, plan and implement the care of patients who are discharged home under the care of the SMART service in the community, review and give care to SMART patients attending SDEC/ Hot clinic for review, administer IV treatments and conduct virtual visits using our remote patient monitoring pathways.

Please note applicants should have a full UK driving license and access to a vehicle, As a Staff Nurse you will provide clinical and professional leadership, supervision and support to patients, and students within the SMART team.
You will triage, assess, plan and implement the care of patients who are discharged home under the care of the SMART service in the community, and administer IV treatments.
The SMART Service is nurse led, therefore the ability to work autonomously as well as effectively within a team is essential. The work will include visiting patient in the community therefore it is usually essential that you have a full driving license.
Members of SMART work autonomously, therefore we are looking for someone who is willing to lone work. Specific training will be provided to facilitate the pathways covered by SMART in-house as well as the community. Confidence, good communication skills and the ability to make sound decisions is also vital.
The Staff Nurse must also be able to liaise with all members of the multidisciplinary team to coordinate safe effective and timely care practices within the SMART service.
The Staff Nurse must also be able to care for OPAT (Outpatient parenteral antibiotic therapy), Heart Failure patients and post-operative patients. Full support and training will be provided to be able to support these patients safely., The post holder is responsible for assessment of care needs of patients, planning programmes of care, implementation and evaluation of these programmes without direct supervision. At times they will have management responsibility for being in charge of the ward and supervising junior members of staff.
The post holder will be a key member in relation to the development of practice on the ward. They will be working independently / autonomously and at times without direct supervision, accountable for their own professional working in a manner that promotes dignity and human rights through the adoption of person centred care principles.
Responsible for carrying out clinical practice within designated clinical areas, ensuring that high quality, current evidence-based nursing assessment, care planning, implementation, interventions and evaluations for patients is provided from admission through to discharge
Will be responsible for the application of knowledge across the range of work procedures and practices, underpinned by theoretical knowledge and practical experience
