35 hrs per week (annualised hours)

Are you passionate about Palliative & End-of-life care ensuring people have a choice about where they wish to spend their last weeks & days of life?

Do you want to further develop your skills & have the time to care for people at home on a one to one basis, supporting overnight in the community? If so we would be delighted to hear from you.

We are looking to recruit a Staff Nurse into our dedicated Supportive Care at Home Team, part of the Palliative Care team for South West Yorkshire NHS Foundation Trust.

The team is unique in its delivery of palliative & end of life care in Barnsley & works alongside other SWYPFT community services & health & social care partners.

We will offer full support & training for the right candidate, this is an excellent opportunity for those Staff Nurses with palliative care experience who have a desire to work in specialist palliative care, full support will be given to the right candidate so that they flourish in this role.

If you are using AI to enhance your application, please disclose this in your NHS Jobs application form., You will work alongside the Supportive Care at Home Care coordinator using your experience & knowledge to take part in assessment, planning of care, intervention & communications with other professionals.

As part of the role & under the direction of service coordinator you will assist in coordinating the service (on a rota basis) & provide overnight support visits with patients & families in their own homes, you will also work weekends & bank holidays.

Successful candidates will maintain high quality care in the community as part of the wider integrated care team.

Enthusiasm & ability to use your own initiative are essential, along with good written & verbal communication skills & ability to manage your time effectively.

You will work both as part of a team & a lone worker, on a night time you will have access to senior members of Urgent care response team for support & advice along with the use of a vehicle to undertake any necessary travel in connection with the duties of the post.
