Full Job Description

UCLH@Home is an NHS service that helps by supporting patients to leave hospital once medically optimised and safe to be managed in the community. Whilst under this service patients remain under the care of their consultant team.

As a member of our Team your role will include direct care delivery within the Patient's own homes. This role will be primarily working within the new Blood Transfusion pathway and so will require an innovative individual used to working in a flexible manner.

An exciting opportunity has arisen to join our UCLH@HOME team as we pilot a new Blood Transfusion pathway.
