Full Job Description

This is an exciting opportunity for a qualified SCPHN Health Visitor/ School Nurse who is innovative, motivated and creative to join the Walsall Healthy Child Programme 0-19 service.

The main role of the Strengthening families lead practitioner is to be the delegated named Health Visitor or School Nurse for children and families requiring specialised evidence-based support. The role will work across the 0-19 age range.

The post holder is responsible for assessing and identification of emerging complex health needs and providing responsive, tailored personalised care for specific caseload of families who have complex needs through a holistic, multiagency approach. As well as contributing to the public health agenda.

The post holder will have in depth knowledge of complexities which are supportive of children and families requiring specialist interventions. They will understand the roles of multi agencies which are available to support and assist families on their own caseload and will provide expert advice and guidance as required, To work closely with multi-disciplinary teams in addressing complex care and support issues across a spectrum of needs, developing positive relationships across the safeguarding partnership including engagement of staff within the HCP 0-19 service.

To provide responsive, individualised packages of care reflecting child/family's needs in partnership with parents and professionals.

To provide one to one support and coordinate the support provided by other professionals to families with complex needs.

To work with parents to empower families to make sustainable changes, ensuring families are fully recognised with appropriate support and challenge to bring about effective change in family behaviours.

To focus on the voice of the child and the family to achieve positive outcomes; with the voice of the child at the centre of care delivery.

To work with family hubs as part of collaborative working supporting children and families with complex needs.

Implementation of parental engagement strategies maintaining parental involvement in care.

To prepare conference reports, attend ICPC's and RCPC's and core groups and other professional meetings, undertaking and fulfilling the role of the Strengthening Families Lead Practitioner.

To support parental resilience and social connections and promotion of positive parenting

To lead others within the Strengthening Families Team providing supervision and guidance in a timely manner.
