Do you have great people skills and love helping those in need? Can you make someone feel comfortable and cared for? Then this role could be for you!

You will be a friendly face to someone who may be struggling to do things for themselves due to age, illness, or vulnerability. You will assess their needs and help them to remain in their own homes, preventing an avoidable hospital admission. If you have a caring, patient, and helpful nature you could make a tangible difference to someone's life. You'll allow them to enjoy healthier, more fulfilling independent lives.

What does a day in the life of a Support Worker involve?

- Providing reactive, focused, and flexible support to people following a hospital stay.

- Assessing service users' needs, completing and following a support plan for them.

- Providing practical and emotional assistance (telephone calls, welfare checks, picking up prescriptions, shopping etc).

A full UK driving licence and access to your own vehicle.

- A good knowledge of services provided by the NHS and Social Care.

- Experience of working with vulnerable people.

- Experience of delivering excellent service to the public.

Please note, there is no personal care of administering of medication required for this role.
