As our Lead Support Worker, you'll be at the heart of supporting our customers who have complex mental health needs to develop their skills to live more independently in the community. Typical day as a Lead Support Worker (known by us as Coordinator)

  • Creating support plans with your customers and delivering support required for customers to meet their needs.

  • We support customers who are open to the NHS Durham and Darlington Crisis teams.

  • Carrying out risk assessments, support planning, goal setting and support sessions.

  • Cuppas, chats and catch ups. There's no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

  • Passion to support our customers to live their best life, working collaboratively with an eye for detail.

  • Experience of creating person-centred support plans and supporting customers to meet their needs

  • Experience of coordinating and assessing customer referrals.

  • The ability to work on your own initiative, remain calm under pressure and have a resilient approach

  • To get from A to B, you'll need a vehicle insured for business purposes. The great news is that we'll pay your mileage!

  • Due to limits on our current certificates of sponsorship, we are not able to offer sponsorship to external candidates for this role. This remains under regular review

    You'll join Lisa and our Durham & Darlington crisis prevention team, we're super passionate about what we do! We're really proud of the support we deliver to our customers and being able to support the NHS crisis teams with their work. We're a supportive bunch and like to have a giggle!

  • Monday to Friday 8am-4pm / 9am-5pm

  • Where we're flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.

  • We primarily deliver our support virtually, but we do spend time at the NHS crisis team office and out and about across County Durham and Darlington depending on the customer needs.

  • Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.

  • You'll need an Enhanced DBS check done and we pay for that.
