As our support coordinator, you'll be at the heart of supporting our customers who have complex needs to develop their skills to live more independently in the community.   • Creating support plans with your customers and coordinating our small team of Support Workers working to the plans.
  • Managing the referrals and liaising with the local authority and other agencies to ensure a thorough assessment is completed, we want to give each customer the best start in our services.
  • Carrying out risk assessments, support planning, goal setting and regular reviews.
  • Support the team with training, sharing good practice and reporting on our KPI's to ensure we provide an excellent service.
  • Cuppas, chats and catch ups. There's no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

  • Passion to support our customers to live their best life, working collaboratively with an eye for detail.
  • Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.
  • Experience of coordinating and assessing customer referrals.
  • The ability to work on your own initiative, remain calm under pressure and have a resilient approach
  • Due to limits on our current certificates of sponsorship, we are not able to offer sponsorship to external candidates for this role. Anna is currently completing her health and social care apprenticeship, Tammy who is our level 3 apprentice and Housing management coordinator, and David our support coordinator who has a wealth of experience in the sector.   • Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
