Come and join Home Group as our Lead Support Worker (known internally as Support Coordinator), where you'll be at the heart of supporting our customers who have complex needs including mental health issues, substance misuse and offending behaviour. You will work with them to achieve their hopes and aspirations while helping them to develop their skills to live more independently within the community. Typical day as a Lead Support Worker Our customers live in our main accommodation as well as living in dispersed properties, where we have 10 flats in the community.

  • Creating support plans with your customers and coordinating our team of Support Workers working to the plans.
  • Supporting our customers who have complex needs including mental health issues, substance misuse and offending behaviour Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.
  • Carrying out risk assessments, support planning, goal setting and regular reviews.
  • Cuppas, chats and catch ups. There's no rushing from one customer to the next here. Plenty of time to collaborate with colleagues too.

  • Passion to support our customers to live their best life, working collaboratively with an eye for detail.
  • Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.
  • Experience of coordinating and assessing customer referrals.
  • The ability to work on your own initiative, remain calm under pressure and have a resilient approach.
  • This position is based within the community, and you will need a vehicle insured for business purposes. The great news is that we'll pay your mileage!
  • About our team You'll work alongside our fabulous team who love to support their customers and help them to achieve their life goals. There are 20 of us in the wider team however our managers are Stacey and Benjiee. Both have worked in various roles within Home Gorup and have 17 years' experience between them. Job details
  • You'll work on a rota basis between the hours of 8am and 10pm and take your turn working days, evenings, weekends and bank holidays.
  • We need you to be flexible to meet the needs of our customers, but we aim to give you the time you need to spend time doing the things that matter to you!
  • Where we're flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
  • Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
  • You'll need an Enhanced DBS check done and we pay for that.
