Full Job Description

An exciting opportunity has arisen for the post of a Support, Time and Recovery (STR) worker on Elm ward at West Park hospital.

West Park hospital is based in Darlington, Elm ward is a 20 bedded adult working age female acute inpatient ward.

The successful post holders will be responsible for working with the MDT for planning and providing appropriate activities to meet the general therapeutic, social and recreational needs of individual service users.

This will need to be flexible and patient centred, changing regularly according to the needs of a patient cohort, and according to the interests of individuals at any given time as well as leave requirements and the need of the ward.

The post holder will do both ward-based related work and also help reintegrate our patient into the local community. Risk awareness is essential for this post. The post holder needs to be enthusiastic and show a good understanding of the importance of their involvement in supporting patients for good mental health.

Main duties of the job

We are looking for people who can work relatively independently, using their own skills to support patients in engaging with meaningful activity, and through this promote wellbeing using a client centred approach.

You will need to be able to communicate well with a wide range of individuals, adapting your communication style according to individual need in a responsive manner.

Main duties will include supporting with the escorting of patients on periods of leave in and around the local area, to their own home, to attend appointments. Transferring of patients to other TEWV sites. Supporting the MDT to and from other sites. Supporting with the delivery of items such as medication or patient belongings.

Providing time to spend with patients in the clinical setting to support with any health and none health related issues such as supporting with phone calls or completing paperwork.

The role is to work full time 5 days a week over a 7 day week, these working days will be depended on the clinical needs to the ward and needs of the patients.

Although this role will primarily focus on STR there will be an expectation for the successful candidate to deliver direct patient care and work directly with the nursing team to ensure Patient care and safety is the main priority. This role will also work closely with our discharge process to help reintegrate our patients into their local community, building support networks outside of the hospital environment.
