Full Job Description

The Forensic Outreach and Liaison Service is a service for Kent, providing a safe, effective and timely pathway from medium/low secure care into the community, for people requiring ongoing specialist forensic mental health care following a secure inpatient admission. The service will provide ongoing care and support for those individuals requiring specialist intervention to address risks and psychological needs and works to support social care needs, which if these failed could potentially lead to relapse and reoffending.
The service is within the South East, Adult Secure Services, New Care Model and will work in parity and close partnership with Surrey and Sussex Forensic Services.
The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010)., The post holder will work within the community forensic and liaison service, based at the Trevor Gibbens Unit with close support from the Mental Health Practitioners and Senior STR worker. This role will be part of a multi-disciplinary team, with a commitment to delivering recovery focused, patient centred care to people with mental disorder/illness, who have offended or are at risk of offending. The service is provided in the community, as well as working closely with inpatient teams in secure hospitals to ensure the safe and efficient discharge of patients to the community., Act as a positive role model for colleagues and others in relation to professional and personal conduct and practice.
Clients:
To establish and maintain effective liaison/communication with service users, ensure that service user's feedback indicates the service is responsive to their needs and is a positive experience. Support individuals within the service, to engage in community activities and education.
Team:
To contribute to the safe and effective function of services by undertaking team-based responsibilities and lead roles, and active involvement during times of service development.
Actively participate in team meetings, caseload reviews and reflective practice.
Implement policies and procedures as directed by the trust/ service director, ensuring awareness of these throughout the team.
