Full Job Description

The Dementia Gateway Service Support Worker role provides person centred support to people who are living with dementia and their carers and families.

Our service is delivered from both a building-based setting in our Dementia Hub and in people's homes in the community.

The Core aim of the service is to deliver beneficial individual activity support to promote social inclusion. We do this in a way that supports all aspects of physical, social and emotional support as required. Most importantly, we do this in a manner which promotes personal independence and individual dignity.

The Successful candidate will have confidence using IT as we aim to promote digitalisation across the Dementia Community and our records are all IT based.

There is a requirement to act as a keyworker and develop individual personal plans for the people we support.

Experience of or enthusiasm for preparing, coordinating and presenting activities is required.

You will have the ability to develop supportive relationships with people living with dementia and their Carers, to enable both to live well through the Journey of Dementia. You will have experience of supporting people to engage including those with complex or anxiety needs or presentations.

The role requires you to work flexibly to meet the needs of people.

An ongoing commitment to support the continuing development of the Dementia Gateway service is required.

The successful candidate will be well organised and self-motivated with excellent communication skills.

Qualified to NVQ Level 2 or an equivalent in Health and Social Care is essential along with a dementia qualification or a willingness to undertake.

You must be willing to undertake training as required for the role, some of which may be in your own time., We are committed to safeguarding and promoting the welfare of vulnerable adults; therefore, the post is subject to medical clearance and an enhanced DBS check.
