Full Job Description

Stockport PCNis in the position to offer exciting new opportunities for Band 2 Support Workers. We are looking for enthusiastic individuals who would work with the Community Nurses and Adults Social care staff to provide a first class service to the residents of Stockport. Line management will be provided by Stockport foundation trust , under NHS terms and conditions. On appointment successful candidates will be expected to complete the Care Certificate.

To work within a multidisciplinary team delivering social and nursing care (appropriate to the post) to clients in their own home. Care delegated by an appropriate practitioner utilising personalised care plans as appropriate.

Working 30 hours per week across the District Nursing Teams as a phlebotomist and other nursing care duties.
