Full Job Description

Stockport PCN is in the position to offer exciting new opportunities for Band 2 Support Workers. We are looking for enthusiastic individuals who would work with the Community Nurses and Adults Social care staff to provide a first class service to the residents of Stockport. Line management will be provided by Stockport foundation trust , under NHS terms and conditions. On appointment successful candidates will be expected to complete the Care Certificate.
To work within a multidisciplinary team delivering social and nursing care (appropriate to the post) to clients in their own home. Care delegated by an appropriate practitioner utilising personalised care plans as appropriate.
Working 30 hours per week across the District Nursing Teams as a phlebotomist and other nursing care duties., To assist with general wellbeing; creating a supportive and re- enabling environment promoting maximum independence. The scope of the role will focus on supporting admission avoidance, early supported discharge and promotion of independence and assisting in the effective management of patients with long term conditions and Palliative patients symptom control.
If candidates who are appointed do not hold the Care Certificate, pay will be abated until completion. The post holder MUST hold a driving licence with access to a car to fulfil this role.

If successful this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary
In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
