Full Job Description

Antill Road is a day service for adults and young people with a range of complex needs including severe learning and physical disabilities, complex healthcare and behavioural needs. In your role as a Support Worker at Antill Road, you will be required to support our customers to take part in a range of fulfilling and meaningful daytime activities both at Antill Road and in the community whilst ensuring that their health, wellbeing and educational needs are met. This will include providing physical, emotional and social care. They will work with customers to promote social inclusion and alongside other members of the team to promote independent living, encouraging customers to maximise their skills and choices., Supporting people with complex disabilities to take part in a range of learning, leisure, health and wellbeing-based activities during daytime hours, with some occasional flexibility required on an out of hours basis
Supporting customers to access leisure and learning-based opportunities both at Antill Road and in the community in line with their individualised activity plan
Supporting each customer in the delivery of their prescribed health, physio, nutritional and sensory routines with guidance from / in partnership with external healthcare professionals
Partnership working with a range of stakeholders including families, healthcare professionals, external personal assistants other support agencies to maximise outcomes for customers
Providing support around personal care and mobility as well as the moving and handling of customers with physical disabilities / wheelchair users
Maintenance of accurate and detailed paperwork for each customer's support file
Participation in support planning and risk management, as facilitated by the lead Support Worker.
Working innovatively and creatively in order to engage hard to reach, chaotic and complex customers.
Observing and monitoring the customer's behaviours, their duration, intensity, as well as factors that may alleviate or trigger the behaviour
Employing superb interpersonal and relationship skills to handle psychological and behavioural changes
Understanding and collaborating with the different support organisations accessible in the community
Presenting and giving feedback to other professionals in a formal setting
Participating in the design and review of behavioural intervention plans
Measuring goals and objectives of behaviour support plans to ensure they are being met
Communicating with families and care managers regarding customer's progress in the absence of management

Supporting customers' physical and mental health

NVQ Level 2/3 or equivalent or experience in the social care/charity sector
Experience working with customers with Autism/Challenging behaviour/learning disabilities
Good IT skills
Training or experience with PBS Approach and Capable Environments framework
