Support Worker

Join our team as a Support Worker for our services across the West Midlands. This rewardingrole involves supporting Service Users to live independently, following personalized careplans from Select Lifestyles Limited.You'll be working at our 6-bed shared house service in Darlaston, helping male clients withday-to-day tasks and activities to support their independence.Responsibilities include (but not limited to):

• Support Service Users in daily living tasks, including domestic chores, meal prep,
budgeting, and community engagement.
• Implement individualized support plans to achieve unique goals and aspirations.
• Provide person-centred care on a 1-2-1 or 2-1-2 basis.
• Maintain accurate records, including Daily Notes, Risk Assessments, Medication
Administration, and Incident Reports.
• Collaborate with professionals, families, and in-house support practitioners when
needed.
