Full Job Description

We have an exciting opportunity for an enthusiastic and motivated Support Worker to join our new Early Supported Discharge service, working with our dynamic team at Gravesham Community Hospital. As a member of the Early Supported Discharge Team for Stroke patients and other Neurological conditions, you will play a crucial role in delivering high-quality care within the community setting. This is an excellent opportunity for a passionate individual to make a real impact in the lives of patients and contribute to the ongoing development of our service. Gravesham Community Hospital is located a short distance from St Georges Shopping Centre and is surrounded by well-known food establishments and local coffee shops making it an ideal location to be based from. This is afull timeposition. The Early Supported Discharge service operates8am - 4pm Monday - Saturday. A full driving licence and access to a vehicle is essential in order to carry out community visits to patients in their homes and visit other bases.,

  • To actively work as a member of the Early Supportive Discharge team for Stroke patients and the Community Neurological Rehab Team to provide a high quality of care to the patient within the community.
  • To actively participate in patients individual rehabilitation / programmes of care under the guidance of an appropriate Therapy/ Nursing professional.
  • To provide and promote a high standard of fundamental care to patients.
  • To work autonomously under the guidance of the Therapist/ Nurse as well as part of a team
  • To provide care to patients in their own homes, group work and inpatient unit- this can include assistance with the provision of personal care tasks for patients in their own home (e.g. washing and dressing, toileting)
  • To work across services with the Community neuro rehab team and the inpatient ward as service demands dictate.
