Full time 37.5 hours per week (Earlies/Lates/Sleep Ins & alternate weekends Must hold a full UK Driving Licence to be considered for this role We have a fantastic opportunity for support workers to join our new team to support the transition of 2 young men who are transitioning from child to adult support, both have just turned 19, have autism and ADHD. This is an exciting time for both young gentlemen, you will be involved in helping them settle in, encourage them to continue and get involved in the activities they love. You will also be supporting a gentleman in his 60's requiring full support with all daily activities. Each of people we will be supporting will have their own private room and shared access to communal areas such as kitchen, lounge, bathrooms and garden. Our aim is to provide them with a stable, consistent and accepting support team with the emphasis on the people we support in gaining confidence and skills to develop their independence in all areas of life. We are looking for enthusiastic, motivated, positive people to support them and want to make it a positive transition for these gentlemen into a life with Dimensions. The Role

  • Supporting young adults with the transition from child to adult support
  • Promoting and supporting their independence within their new home
  • Supporting on a 1-1, 2-1 basis, dealing with behaviour and emotional distresses due to past trauma
  • Going out, xbox, skateboarding, theme parks, martial arts
  • Watching and taking to football games - big fans of Portsmouth
  • Support with developing friendships and connecting with the local community
  • Providing emotional support, teaching daily living skills

    Ability to see past presenting behaviours and to connect via a variety of means, including social life skills, therapeutic activities and participation in effective role modelling.
  • Ability to work as part of a dynamic team offering support to three young men in their home.
  • Focus on each person supported development and needs and put these centre to their support plan including nutritional, medical and hygiene guidance and support.
  • An enthusiastic and proactive approach to promoting and supporting current and future opportunities for work placements and further education along with hobbies and interests.
  • Ability to provide varying degrees of support in daily tasks within the home, to support and promote the development of independence of the people we support.
  • Ability to liaise with the variety of professionals and outside agencies and be able to undertake a variety of administrative tasks, including accurate recording and reporting using various IT platforms.
