Full Job Description

  • Work collaboratively with the people we support to create and review Risk Assessments and Support Plans in line with Hounslow Reach service timescales


  • Work in partnership with Hounslow Reach colleagues and other services, including health, housing, social care and Employment & Skills teams, to deliver holistic, co-ordinated support to clients


  • Contribute towards service evaluation and improvement through engagement with the people we support, with colleagues, and with other key stakeholders

    Experience in mitigating and reducing risk to clients caused by health, safeguarding or welfare concerns


  • Ability to be open to learning and using Trauma-Informed and Strengths-Based approaches to motivate and enable the people we support to achieve their goals and develop their resilience


  • Experience in being reflective, flexible and responsive in meeting the needs of the people we support


  • Ability to be confident in advocating for the people we support through joint work with partner services


  • To promote, and positively represent, Thames Resch in meetings and events with external services


  • Experience in organising and managing your time to meet the needs of the people we support, Disability Confident
