As a Support Worker in our Mental Health Outreach Service, you can make your own powerful contribution to a diverse team who share the same values and vision. You'll be a valued member of a service where you can make a real difference in the lives of people with mental health challenges. With wellbeing at the heart of everything we do, we work to empower people in achieving their goals. Joining a team where everyone feels comfortable showing up to work as their whole self, you'll work with colleagues and teams who are involved in the care of your assigned service users, to make sure the best possible service is delivered.

As a key worker for users of our Mental Health Outreach Service, you'll identify their needs and work with them to develop care plans focused on achieving life-enhancing outcomes. It's all about promoting independence and emotional wellbeing. You will develop respectful, supportive and sensitive relationships with service users. And you'll assess, plan, implement, manage and evaluate activities within a person-centered planning framework, which will help people develop valuable life skills. Ultimately, you'll help people improve their mental health and wellbeing, while securing more control over their own lives.

To make a difference from day one, you'll need some experience in a health and social care role. You have worked with people who have severe and enduring mental health problems, and with their family and carers. You demonstrate an understanding of key principles which underpin the way we work; social inclusion, independence, empathy and empowerment. As well as the ability to plan and deliver varied activities, you are comfortable producing written reports and statistical data which show the difference we can make.

Mrs D was frail and unwell. Her son was her main carer, but he wasn't coping. We stepped in to help, but they were hostile to outside agencies. They'd always lived together and were terrified they'd be split up. Our social worker did a great job persuading Mrs D to get hospital treatment and a temporary stay in a care home. In an epic effort throughout COVID, she gained the son's trust and sorted all the repairs, carers and nurses needed so that Mrs D could come home to live with her son. We also got Mr D regular respite and a personal budget awarded in recognition of him as her carer. After all, there's nothing more important than living the life you want., Westminster is an amazing place.
