Full Job Description
Prevention services support vulnerable people by providing them with skills, housing advice, accommodation, and tenancy sustainment support. All these services to help people transition towards leading independent lives.
Thames Reach welcomes applications from people with transferable skills.
We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions.
You will:
.Conduct regular assessments of service users' needs to tailor support effectively and ensure progress toward their goals.
.Collaborate with other departments to ensure the highest quality of care for service users.
.Develop SMART goals (Specific, Measurable, Achievable, Relevant, Time-bound) and assist service users in reaching these objectives.
.Promote engagement and social inclusion, while fostering autonomy and independence, to enhance overall physical health outcomes.
.Provide a range of interventions, both short-term and long-term, tailored to the individual needs of each service user.
.Record keeping ensuring that the required paperwork and monitoring is completed including the updating of case files to the standards set out by Thames Reach.
Experience in supporting service users with mental health challenges, demonstrating empathy and understanding.
.Strong prioritisation skills and the ability to manage your workload independently and efficiently.
.Proficiency in Microsoft Office applications, including experience with databases to track and manage information effectively.
.A proactive approach with keen attention to detail, ensuring high-quality support and documentation.
.Excellent interpersonal skills to develop and maintain positive working relationships with service users and colleagues.
.A genuine interest in mental health and a commitment to making a positive impact on the lives of individuals facing both short-term and long-term challenges., Disability Confident
