Full Job Description

We are excited to advertise two posts, for experienced Surgical and Urology Advanced Care Practitioner's (ACP's) you will need to be qualified and practising autonomously as an advanced clinical practitioner. As these are new roles in the Trust and Division, you will be pivotal in development of this post. The post holder will demonstrate a high level of expertise within the specialist service providing advice, education and support to staff, patients, their families, and carers. This will encompass the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice., You will lead the take in SDEC, directing the juniors whilst reporting to the on call registrar and Consultant, day to day. Triage SDEC GP referrals and explore alternative pathways to avoid UEC attendance eg navigate to hot clinics We anticipate that this role will develop in time, with the possibility of minor op's and hot clinic reviews, being undertaken by the ACP. Support new ways of working that emphasises a more efficient and patient focused service, and will ensure the safe treatment, referral and discharge of patients with undifferentiated and undiagnosed presentations in their area. Responsible for leading the on-going development of clinical practice and standards of care within the service, including the development of policies, procedures, protocols and guidelines in collaboration with multidisciplinary colleagues
