Full Job Description

The LiveWell team are looking to recruit a team leader to work on a new and exciting project to support residents of Cardiff and the Vale at risk of Falls. The main duties and responsibilities will be around managing the Community Falls programme and supporting the MDT to deliver high value high impact interventions. The post holder will work collaboratively with CRT's and community partners across the health boards geographical area. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
