AN EXCITING OPPORTUNITY HAS ARISEN FOR A TEAM LEADER TO WORK WITHIN THE COMMUNITY BEDS TEAM

The team provide a community bed-based rehabilitation service across three community care home settings in the Nottingham City locality, often patients are admitted to the community beds directly from hospital, but some are admitted directly from their own homes after experiencing a crisis that does not require acute secondary care. The team are based within the care homes and complete assessments, rehabilitation, and discharge planning in a timely manner to support patients back into the community.

The successful applicant will, alongside the team manager, have the day-to-day operational responsibility to ensure the delivery of an efficient and effective community beds service in line with local, national and professional agendas and clinical governance.

The applicant will work closely with the Team Manager and Clinical Lead to create a strong leadership team, being a visible and encouraging leader to a large team of multidisciplinary professionals, aspects of the role include but are not limited to; rota responsibilities, managing sickness and absences in line with local policy, ensuring compliance is up to date in regarding to staff training and developing, having an overview of staff appraisals, data collation and audit responsibilities.

This post requires the candidate to work between the hours of 8pm-4pm Monday to Friday, The post holder will have day to day responsibility for the operational management of the community beds team under the supervision of the Team Manager, alongside this the post holder will be responsible for ensuring the operational effectiveness and adherence to quality standards and key performance indicators for all external contracts/ internal service level agreements as well as marketing and ongoing development of the service. The post holder will also lead on partnership communication and initiatives with internal and external stakeholders and other third party organisations.

The successful team leader will play a key part in supporting a large multidisciplinary disciplinary team of physiotherapists, occupational therapists, nurses, assistance practitioners and rehab support workers. The teams plays a pivotal role in supporting local acute trusts in the discharge of patients who require a period of rehabilitation and/or assessment, this can come with some operational pressure, the team leader is required to be able to maintain a busy schedule, prioritise workloads and provide support to the team as required.

The candidate will be required to have excellent communications skills, written and verbal, responsibilities will including data collection and report generating, taking part and lead on meeting which will include. communicating with internal and external agency.
