Full Job Description
The borough of Ealing has a diverse range of communities and population with unique health and social care challenges. The Healthy Ealing programme is a new initiative aimed at enhancing the health and social care outcomes for the residents of Ealing.
Healthy Ealing Team Manager will play a pivotal role in the ongoing development and operational management of the Healthy Ealing programme. This position requires someone who can drive the programme's objectives, ensuring that it effectively meets the needs of the Ealing community.
The aim of the role and the programme is to improve health and wellbeing in local communities by improving access and support for the building blocks of health, upskilling the workforce to support local communities in a culturally competent manner and running events to reduce health and social inequalities in Ealing through structured public education, raising awareness and signposting.
The team will be supporting the Ealing Borough based Partnership in the delivery of community outreach events and public health collaboration, fostering relationships across the borough to build capacity for equity and inclusion by planning, coordinating and facilitating ongoing conversations and projects around health equity and social determinants of health.
Main duties of the job
This role will be key in leading the delivery of an integrated and accessible health and social care system to improve equity and reduce variation for the populations we serve. Specifically, the role will:
Lead the Health Ealing Team within Ealing
Be the coordinating function to bring together a wide cross-section of stakeholders to run health outreach events for local targeted population groups, including (but not limited to: asylum seekers, people experiencing homelessness, vulnerable migrants, gypsy/Roma communities).
Analyse and interpret national and local data and national evidence of effectiveness using this to developed targeted local projects
Assist local partners in assessing of health and social care variation, system challenges and capacity needs to develop fully embedded health equity programmes
Mobilise and provide education to local partners around social determinants of health and health equity within Ealing.
Organise, conduct and facilitate improvement planning in line with health needs assessments for the communities of Ealing
Engages diverse community partners in developing, implementing, and evaluating policies, programs, and services that affect the health of communities' Ealing wide
Build relationships with and engage trusted voices within the targeted community groups to disseminate information about events, equipping them with skills and providing connections with services to facilitate an improvement in their Health & Wellbeing.
