Full Job Description

An opportunity has arisen for a highly skilled, enthusiastic and motivated individual to join the management team of Integrated Care Services across Nottingham City.

The Post holder will be responsible for managing the Clinical Team comprising of multi-disciplinary professionals delivering high quality Reablement and Urgent Community Response ( UCR) services.

The post holder is also responsible for managing the Community Triage Hub (CTH) which works alongside the clinical teams in the Community and within the Acute setting.

As the Team Manager you will be responsible for leading all aspects of strategic and service delivery of the busy and dynamic team. The role includes delivery of efficient and effective services in line with local, national and professional agendas.

This post involves managing staff working in community, monitoring performance, managing change and ensuring contractual targets are met.

A good understanding and experience of Health and Social care provision and ability to work closely with partner organisations and other stakeholders is essential.

The hours of work will be 37.5 per week., The successful candidate must demonstrate good operational management, ability to facilitate service development, manage competing demands and have excellent leadership skills. Change management is also a key requirement, as the services continue to transform while we move forward with the health and social care agenda.
