Full Job Description

An opportunity has arisen for a highly skilled, enthusiastic and motivated individual to join the management team of Integrated Care Services across Nottingham City. The Post holder will be responsible for managing the Clinical Team comprising of multi-disciplinary professionals delivering high quality Reablement and Urgent Community Response ( UCR) services. The post holder is also responsible for managing the Community Triage Hub (CTH) which works alongside the clinical teams in the Community and within the Acute setting. As the Team Manager you will be responsible for leading all aspects of strategic and service delivery of the busy and dynamic team. The role includes delivery of efficient and effective services in line with local, national and professional agendas. This post involves managing staff working in community, monitoring performance, managing change and ensuring contractual targets are met. A good understanding and experience of Health and Social care provision and ability to work closely with partner organisations and other stakeholders is essential., The successful candidate must demonstrate good operational management, ability to facilitate service development, manage competing demands and have excellent leadership skills. Change management is also a key requirement, as the services continue to transform while we move forward with the health and social care agenda.,

  • To provide effective leadership and management support to our Clinical Team comprising Integrated Reablement, Urgent Community Response (UCR) and Community Triage Hub services.
  • To drive Change management through the service functions within established service practice and across new business opportunities as the focus moves from specific services to community bed pathways and outcomes, including the urgent care agenda.
  • To develop a high-performing team culture aligned to organisational-centric objectives, focus and behavioural outcomes across the service line and through strategic and operational plans.
  • Understanding the workforce infrastructure to recruit, engage and retain.
  • To lead and facilitate a culture of continuous improvement across the services in line with strategic and operational plans.
  • Dimensions
  • The Team manager will be responsible for leading the At Home Clinical team comprising clinical and non clinical team leads, ACPs, AHPs, Nurses, APs and administrators. This role will involve working along with other team managers in CityCare to provide the best outcomes to our patients.
  • To performance manage services and staff in accordance with organisations objectives, KPIs and performance data.
  • To support the delivery of CityCares statutory returns (ICB,CQC, CiC Regulator, Auditors), Strategic Direction
  • To work with the CDGs, Commissioners, System partners and the urgent care system to influence and implement the direction of travel across service functions reflecting local and national policy direction.
  • To demonstrate operational output from strategic direction and take the organisation forward in todays commercial market by influencing and implementing service transformation and improved patient outcomes, being prominent in the leadership of the cultural change required to transform the development and delivery of community health services and build healthier communities.
  • To ensure performance targets and strategic objectives, are translated into operational realities and are embedded in staff roles and functions therefore supporting CityCare as a key player in the provider area.
  • Bring alive the clinical workforce planning activities and the modernisation and re-design agenda amongst front line staff.
  • To develop with other senior directorate managers appropriate mechanisms and structures for the planning and implementation of service expansion and ensure services are designed around service users and commissioners.
  • Developing networks through working in partnership through the Urgent care system, CDG development and stakeholder engagement, encouraging contribution and creating an environment where others have the opportunity to contribute.
  • To participate and lead in conjunction with business partners in contract service reviews and service negotiations and direction as appropriate.
  • Operational Management
  • To actively demonstrate authentic management credibility to ensure appropriate governance measures and processes eg clinical / managerial supervision, essential training, risk management and support are in place and maintained across locality team which in turn drives and delivers board assurance.
  • To have a lead role in supporting staff to think family and link closely with safeguarding lead and team.
  • To ensure that the appraisal process is cascaded through the community team and individual objectives are identified and actioned that are strategically linked through your span of control.
  • To ensure professional development opportunities exist for locality team and self.
  • With the Integrated social care managers develop a training and development / workforce plan for service areas that cross team boundaries, developing a flexible responsive workforce and review accordingly.
  • Act as a role model for excellence in line with the CityCare values and Brand
  • Lead on the recruitment, retention and succession planning of staff within clinical team in line with the structure.
  • To ensure that all relevant policies and procedures are relevant, contemporaneous, and adhered to by all staff.
  • To ensure appropriate incident reporting and subsequent action planning and management takes place.
  • To manage the relevant service budget to ensure financial targets are met.
  • To develop close working relationships and good lines of communication with the Urgent care system, locality social services, CCG, primary care and other key locality stakeholders
  • To work with the CityCare internal support services, finance, business, HR, workforce in developing and maintaining effective information systems across the related services.
  • Ensure the performance management of services against the required KPIs are maintained and analysed and acted upon.
  • To ensure that clinical care, planning and provision is integrated across the Urgent care system, CDG and that partnership working with key locality stakeholders is promoted.
  • To work closely with the integrated health & social care managers / business manager / Commissioners to ensure service specifications are developed which support the efficient and effective delivery of locality-based services.
  • To work with key stakeholders to ensure that locality-based pathways of care are delivered as seamlessly as possible and provide truly integrated health and social care services.
  • To support the involvement of local people in the development of services by ensuring mechanisms for PPI and patient feedback across community services.
  • Along with other leads participate in regional and national networks.
  • Identify, manage and monitor clinical risks in conjunction with the governance and patient safety team.
  • Develop clear plans to meet quality requirements of external agencies CQC and all CQUIN targets.
  • Lead on audits and ensure all agreed outcomes are actioned.
  • To take on any additional duties as required by senior managers.
  • To work with Clinical staff and Integrated Health and Social care managers to achieve service goals, managing team resources which are available and using influence to ensure that resources are used efficiently and safely and reflects the needs of the organisation.
  • As a leader and manager you will work corporately and collaboratively to promote the future success and sustainability of CityCare.
  • To represent CityCare internally

    Degree in health-related qualification OR relevant experience of leading and managing a team.
